4/12/2023 EnchantaVaughn Approved 1. Behavior: Upon arrival to the home setting, QMHP-C was informed by Ms. Hodge Ashiyah has been engaging in profanity and not wanting to listen to adults. She continued to report she receives calls from school as it relates to her level of disrespect for those who are in authority. Ms. Hodge discussed with QMHP when in the home setting, she has to redirect her several times to complete simple task within the home (i.e. clean her room, take out the trash, and be home by curfew). 2. Behavior: Ashiyah and Ms. Hodge appeared to be in a content mood as they both engaged in the family session. Ms. Hodge indicated she tries to teach her children right from wrong now because she does not want them to make the same mistakes she made growing up. Ashiyah appeared to be frustrated during the session when discussing respecting those who are in authority because she stated, âI give off the same energy other give to meâ. 1. Intervention: Todayâs session QMHP-C engaged Ashiyah and Ms. Hodge in a family session to assess both of their moods and to continue to discuss with Ashiyah on the importance of adhering to those who are in authority in the home, school and in the community setting and being able to control his anger in an assertive manner. QMHP-C conducted todayâs individual counseling session to assist Ashiyah to express feelings of anger through appropriate verbalizations or healthy physical outlets on a consistent basis. QMHP-C initiated a discussion on goal setting; examples were provided to Ashiyah to continue to assist her with being able to communicate effectively to her mother and to QMHP-C with situations that makes her upset or those negative behaviors that leads her to displaying inappropriate behaviors. 2. Intervention: QMHP-C discussed Ashiyahâs continued needs to accomplish goals while maintaining an unconditional positive regard. QMHP-C commended Ashiyah for making progress towards some of her goals; however, she was encouraged to continue to work towards mastering strategies identified during sessions. QMHP-C provided Ashiyah and Ms. Hodge with support through active listening, paraphrasing and summarizing what was being discussed with the QMHP-C. 1. Response: She displayed a positive demeanor throughout the entirety of todayâs session. She demonstrated age-appropriate communication skills; she spoke in appropriate tone and provided positive feedback to conversation. Ashiyah conveyed prior knowledge of learned strategies to address each goal mentioned by QMHP-C; she stated that she implements learned strategies when necessary however she has difficulty implementing strategiesâ when angered or frustrated. 2. Response: Ashiyah stated to QMHP-C that she got loud with her mother because she doesnât know how to talk to people with raising her voice and if she doesnât want others to be nasty with her then she doesnât need to be need to be nasty with others. Ms. Hodge stated to QMHP-C and Ashiyah that it appears as though when she speaks with a nice tone, Ashiyah donât seem to understand that; however, when she speaks in a high tone everyone in the home seems to understand that she means business.
